Driving Health Outcomes in Northeast Ohio

The Northeast Ohio Quality Improvement (NEO QI) Hub is led by Case Western Reserve University (CWRU) in partnership with Northeast Ohio Medical University (NEOMED). We work alongside Ohio’s regional QI Hubs to support clinicians, their practices, and their patients with an evidence-based QI framework.

Northeast Ohio's industrial history and economic shifts have led to pockets of severe poverty and limited healthcare resources throughout Cuyahoga, Summit, Lorain, Mahoning, and Trumbull counties. Urban areas struggle with higher rates of chronic diseases, while rural areas face barriers like fewer healthcare providers and transportation challenges. By targeting these specific needs, the NEO QI Hub aims to reduce disparities and improve health outcomes through community partnership and investments in interventions that are designed to improve chronic disease.
