Primary care practices are invited to participate in the Achieving Health Equity in Diabetes initiative (AHEAD), aimed at improving A1C control and eliminating disparities in diabetes management.
Practices participate in a kick-off meeting, monthly webinars, and QI coaching, to develop rapid Plan-Do-Study-Act (PDSA) cycles, and implement evidence-based interventions designed to impact the key drivers of A1C control.
Practices receive a participation stipend and have access to free CME and MOC.

Practices participating in the AHEAD Initiative will:
Implement proven strategies with expert guidance and tools to improve diabetes control
Receive graphs for continuous QI
Provide a structured scholarly activity for residents
Partner with Managed Care Plans, patient advocates, and community-based organizations
Align with value-based payment metrics
Gain access to free CME and MOC
